COVID-19 Vaccination clinic – Pfizer 6 months-adult / Moderna 6 months - 5 years
Pfizer vaccines are available at this clinic to everyone age six months and older. First, second, third and booster doses are available. This clinic also accepts walk-in appointments for those who are eligible for a 2nd booster dose. Please do not make an appointment if you wish to receive a 2nd booster dose.
The Moderna vaccine for children ages six months to five years is available at this clinic. The Moderna vaccines are NOT available for anyone older than 5 years at this clinic.
Recommended childhood vaccines are also available at this clinic.

Walk-ins are welcome and are asked to arrive at least one-half hour before the clinic is scheduled to close.
All persons must show identification. For children 17 years of age or younger, a parent or legal guardian must provide consent for vaccination, either in person, by phone at the time of the vaccination appointment, or by completing a consent form. For minors who are six months 15 years of age, additionally, an adult caregiver must accompany the minor. Link for consent form.
Note to Parents: Your child will need additional doses of the COVID-19 vaccine depending upon your child’s age and the vaccine given. Medical practitioners will be on-site at all times at each clinic to answer any questions and tend to any needs or concerns.
